Preschool Parent Handbook

The Preschool Parent Handbook (PDF) answers many of the Frequently Asked Questions about the school's policies, procedures and programming.  If you have a question that's not answered in here, please email the Director. 

Required Forms for Current Students

Preschool Enrollment Packet (part 1)
Includes the Tuition Schedule, Enrollment Contract, Authorization for Deposit Payments and Authorization for Recurring Payments required to confirm enrollment (for families accepted off the waitlist for the 2008-2009 school year).

Preschool Enrollment Packet (part 2)
The second part of the forms required for each child: Emergency Information Form, Permission to Take Off School grounds, Authorization for Child's Emergency Medical Treatment, Membership Letter, Enrollment Information Sheet and the DC Health Certificate.

Individual Forms:

• DC Health Form for Preschoolers
  DC has created a new form which goes into effect June 30, 2009.  The above link will take you to the new form.
  This form must be updated annually and needs to show the results of TB testing, lead screening and an immunization history.  Please make sure that it is dated!

New DC Immunizations Requirements (as of June 30, 2009 and the additional changes for the 2009-2010 school year) (link opens a PDF)

Please note the following changes to the DC Immunization Clinics (the "minute clinics"):

Effective February 1, 2009, the following District of Columbia Immunization Express Clinics will be CLOSED:

1. Immunization Express Clinic at the Reeves Municipal Center
2. Immunization Express Clinic at the D.C. Birth Center
3. Immunization Express Clinic at the Congress Heights Clinic
4. Immunization Express Clinic at the Israel Baptist Church

Georgia Avenue Express Clinic will remain OPEN Mondays and Tuesdays, 10:00 AM – 2:00 PM for UNINSURED and UNDERINSURED residents
